2011-2012 Registration Procedure
Thank you for your interest in registering for Care After School.
To expedite your registration, please follow the steps listed below:
1. COMPLETE THE APPLICATION:
- Complete all information on all 2011-12 registration forms (6 pages: application form, child enrollment and health information, permission form, and letter of agreement).
- INCOMPLETE FORMS WILL NOT BE ACCEPTED. Please be sure all forms are filled out completely.
- Please note: A child cannot be enrolled at CAS without permission for emergency transportation. To grant permission, please complete “Give Permission to Transport” in the Emergency Transportation Authorization section of the registration form.
- Print forms. Sign and date pages 1, 4,5 and 6
2. SUBMIT THE APPLICATION:
- Enclose a CHECK or MONEY ORDER, made payable to Care After School, for the $30 per family NON-REFUNDABLE REGISTRATION FEE. Please include your child’s name and school in the memo portion for your check or money order.
- Return the completed application and check, by MAIL ONLY to the address on the Registration Form. INDIVIDUAL CAS SCHOOL SITES CANNOT ACCEPT APPLICATIONS.

Registration Forms
You will need to download, fill out, then print the registration packet. Please read the Parent Handbook before completing the forms. The Tuition Express application is recommended, but not required. You will need Adobe Acrobat Reader to view the forms. Acrobat Reader is available for free.

Notification
If space IS available, a confirmation will be mailed at which time the first monthly payment is due to secure enrollment in the program.
If space is NOT available, a confirmation will be sent indicating your child’s place on a waiting list. You will be notified as soon as an opening is available at which time the first monthly tuition payment is due to secure your enrollment in the program.
If the waiting list is full, your application and check will be returned. This is the only circumstance when the non-refundable registration fee is returned.
Spaces Available
Spaces are available at Liberty, and all morning programs (AM programs are held at Evening Street, Liberty, Slate Hill, Wilson Hill, Worthington Estates, and Worthington Hills).
We are accepting application for the waiting list at Brookside, Colonial Hills, Granby, Slate Hill, Wilson Hill, and Worthington Estates.
No more applications will be accepted for Bluffsview, Evening Street, Worthington Hills and Worthington Park. Those programs and their waiting lists are full.
Please call the CAS office at 614-431-2266 for up-to-date information since the number of spaces available changes quickly. Our office hours are 11AM to 3PM on school days.
To Withdraw...
If you find that you no longer need the services of Care After School, please note that CAS must be notified in writing, by August 15th in order to have your first tuition payment refunded. No refunds will be made for withdrawal from the program after August 15.
After school starts in August 2011, CAS must be notified in writing by the 15th of the month to withdraw from the program for the following month. If notice of withdrawal is not given by the 15th of the month, half the next monthly payment is required.

To Change Days of Attendance
CAS must be notified by the 15th of the month to change your child's days of attendance for the following month. Due to the great demand for full-time spaces, part-time spaces are limited in number. Full-time participants may drop to part-time attendance only if openings for the selected days are available in the designated time spaces. Likewise, part-time participants may change to full-time if an opening is available to the designated full-time spaces. Please call our office at 431-2266 as soon as possible to request a change. If space in the program permits, the change will be effective after the next monthly payment.
